There are several ways Multiverse stories can be handled:
Main continuity and (almost) no Multiverse stuff.
Different realities.
Alternate timelines (with possible futures or alternate history).
The case you mentioned seems to be the last one.
When it was released, Infinite Ending was considered part of 616. After some discussion, they removed it from 616. The discussion wasn't acceptable to all parties, so some people still hold on to the original idea.

Jim Starlin's Thanos stories should be regarded with a grain of salt because he just REALLY likes Thanos for some reason, this is a guy that got mad that Thanos lost in Avengers Endgame even, Starlin’s work completely ignores the established rules for the Tribunal, TOAA and all sorts of stuff. Most importantly Marvel's Executive Editor Tom Brevoort has stated on his Tumblr blog that "Marvel: The End" (and all other "The End" series) is not in any way a part of the official Marvel continuity. So all naysayers can-
The continuity changes all the time tho with smaller things
And Tom just doesn’t like Jim. I don’t think one writer can just say someone else’s comics are non-canon just because they’re an editor, I think it’s more up to the reader.
I don’t think anyone in marvel can really deem something canon or non-canon, and it’s up to us to decide for ourselves what is and isn’t.
And I mean sure maybe the writing was crappy but I don’t think it should be disregarded completely just because one writer deems it non-canon.
Im not referring to it being canon with earth-616 either, I’m just asking whether it is part of the multiverse as a whole.
As well as I’m not talking about marvel the end either, I’m talking about the infinity siblings/infinity conflict/infinity ending trilogy, and whether it is part of the multiverse/omniverse or not, not whether it’s canon with 616.
