As Spider-Carnage.
Ben Reilly's past is presumably the same as that of his Earth-616 counterpart.
This version of Ben never appeared canonicly in the game, only as a costume for Spider-Man, the main difference is that he was bonded with Carnage instead of Venom.[1]

Seemingly those of the Benjamin Reilly of Earth-616. While bonded to the Carnage symbiote, he was even more powerful and had access to symbiote-styled attacks.
