As the world mourns the apparent death of Captain America as announced by Black Widow, Cap himself revives and bursts free of a glass tube inside an unknown laboratory. A female voice taunts him with past failures; he turns a corner to discover it is Sharon Carter, whom he thought dead. She is cold to him, but reveals that a third party teleported her and some medics into Avengers mansion just before he died, putting his body on ice and transporting it away to a secret lab, where a total fluid transfusion was performed. This makes him wonder if the super-soldier serum has been flushed from his body. She sets a team of fighters against him to test him; they defeat him making him realize his body is much weaker than before.

Sharon is surprised at this but says the mission must continue and teleports with him to a factory outside of Green Bay. They are accosted by guards, and Steve’s strength slowly begins to return as they fight. He realizes that if the super-soldier serum is still in his blood, this can only mean his blood transfusion came from the Red Skull, and turns to find him standing there. He tries to throw a punch but gets planted on his ass by the Skull, who says he saved his life because he needs his help against a common enemy. Steve demands to know who this enemy is, when suddenly the abandoned factory is replaced by an enormous, hi-tech facility appearing in front of them, and Sharon says they are facing World War III.

Synopsis for "The Devil You Know"

Sharon Carter and the Red Skull fill Captain America in on the Kubekult, a faction of neo-nazis and AIM members that had stolen the Skull's ornamental Cosmic Cube containing Hitler's soul. They were the ones behind the recent Cosmic Cube flares on AIM Island as they experimented towards their goal of re-writing reality. Skull makes that they have to work together against this common enemy, though Cap can tell right away that he just wants the newly functioning Cosmic Cube for himself. While they pursue the Kubekult members to a new location, Cap also hears from Sharon Carter what happened to her since her apparent death. SHIELD deployed her on an undercover mission and then she lost contact with them. She was forced to fend for herself as a deep cover mercenary for far too long and formed a resentment for the government systems she used to rely on. While her attitude remains cold, she does start to accept that Steve truly believed she was dead and that he didn't abandon her like SHIELD did.

Sharon's intel leads them to a government research facility. Cap being forced to fight through soldiers alongside the Red Skull does not help convince them that he's the real deal. When they catch up to the Cosmic Cube, it's in the middle of being activated. Sharon dives for it, but then she vanishes along with the Cube just as its waves transform the American soldiers into Kubekultists.

Synopsis for "Triumph of the Will"

The Cosmic Cube has transformed a base full of American soldiers into those of the New World Reich. While fighting through them, Captain America feels his full strength return in time to save the life of the Red Skull, a deed he considers makes them even for the Skull reviving him. Cap eventually catches up to Sharon, who has taken the Cube and is declaring that the only way to stop Hitler's reality from taking over is for someone else to use the Cube to override it with their own. Steve disagrees with imposing any one person's ideals on reality this way, but soon realizes he needs to act to keep the Cube away from the Red Skull either way. He's trying to make the Cube destroy itself when a nearby explosion jars it from his hand and it falls towards the Skull.

Captain America reaches and stops the Red Skull in time and the Cosmic Cube is destroyed. He's then congratulated by his longtime faithful companion, Bucky Barnes, an inconsistency only possible because Cap is now trapped in an illusion created by the Red Skull now holding the Cosmic Cube in triumph.

Synopsis for "Operation Rebirth: Conclusion"

The Red Skull has trapped Captain America inside the Cosmic Cube, counting on him to defeat Hitler as he was created to do and then naturally surrender the Cube's power to the stronger will of its holder. While the two minds are in opposition, the Cube's powers are neutralized, forcing the Skull and Sharon to fend off the Kubekultists on their own while they wait for something to happen inside the Cube. Sharon makes it clear to the Skull that she won't let him keep the Cube either way, so he turns on her knocks her aside.

Within the Cosmic Cube, Steve is living out a dreamlike reflection of his life that mixes events of World War II with elements of the modern age and with happier endings to all the tragedies he had faced since becoming Captain America. Bucky Barnes is at his side the whole way. While he resists seeing it for the illusion it is, eventually his subconscious comes out speaking through the form of Bucky and convinces him to stop living in the past and embrace reality. The revelation gives Cap the power to escape the Cube, much to the surprise of the Red Skull. The Cube is knocked from his grasp but the Skull dives for it once more. Cap throws his shield to sever the Skull's arm and shatter the Cube. The Red Skull vanishes in the resulting burst of energy and the Cube's effects on reality are all reversed.

Afterwards, Sharon agrees to go with Steve to SHIELD to find out why they allowed her to be presumed dead for so long.
