Synopsis for 1st story
Kneeling before Cletus Kasady, Kenneth Neely introduces himself. Smirking, Cletus asks if Kenneth's name is supposed to mean something to him, then scoffs at Kenneth saying that he's like Cletus. Annoyed by Cletus calling him "Ken", Kenneth insists, saying that he's an artist who's transformed thirty-three people into beautiful canvases of blood and bone. Yanking his hand away as Kenneth attempts to kiss it, Cletus uses the Extrembiote to transform their surroundings into a truck stop in Iowa strewn with the dozens of corpses, boasting that he slaughtered thirty nine people there a few years back simply because he got bored waiting for his pancakes. Amazed, Kenneth touches one of the corpses and is surprised that the blood tastes real, asking how Cletus is doing this. Thinking on it, Cletus comments he thinks he killed the man with a slotted screwdriver. Kenneth attempts to clarify what he meant, Cletus interrupting to say that within St. Estes Home for Boys he can do anything he wants. Taking offence to Kenneth's claim that they're alike, Cletus causes the corpses to rise up and restrain Kenneth, who stammers that he came to ask Cletus' help. Seething, Cletus produces a bloodstained slotted screwdriver and reiterates Kenneth's claim before declaring his intent to slowly, painfully, and thoroughly erase him from existence. As the screwdriver inches towards his eye, Kenneth exclaims that Carnage is becoming a god and has to be stopped.
On Nidavellir, Jon Shayde laments having become the apotheosized Carnage symbiote's new host, despairing that his work as a homicide detective already taught him all about the most vile traits of humanity. Refusing to give in, he pries its jaws open and attempts to free himself from it, but the symbiote mockingly sneers that this was inevitable. As Jon protests that he's a good man, the Carnage symbiote replies that he's curious -- or else he wouldn't have followed it across the cosmos -- and that it's too late to go back now. Shoving Jon back into its maw and engulfing him once more, Carnage picks up All-Blood and says that now they can have a civil conversation on their way to their next target. Aghast, Jon asks how much more power the symbiote needs, Carnage retorting that it has a radiant and noble goal in mind but that to attain it they'll need to journey elsewhere. As Carnage conjures a portal to the Spotted Dimension, Jon feels it rooting around in his memories and demands to know what it's doing; Carnage saying it's seeing all the curious places he's been to in his pursuit of it so that they can revisit them.
Seated in the diner, Cletus helps himself to a stack of pancakes and asks if he heard correctly that his former symbiote is currently bonded to a policeman. Kenneth confirms this and is taken aback that Cletus is amused by this development, exclaiming that Carnage is going to slaughter the gods and douse the stars in their blood if he doesn't come with him right away. Ignoring him, Cletus remarks that the truck stop's pancakes were exquisite but is frustrated that he can't quite get the flavor right with his recreations. As Kenneth angrily demands Cletus take him seriously, Cletus abruptly stabs his hand with his fork. As Kenneth recoils in pain, Cletus mocks him before licking the blood off, saying that Kenneth has yet to say anything that could make him want to leave his lair. As their surroundings change, Kenneth asks where they're going; Cletus saying Kenneth should tell him since it's a scene from his memories. They find themselves in the Southbay Industrial District fur factory where Kenneth left his last victim. As Kenneth recognizes the scene from months prior, Cletus points to Kenneth's past self hard at work arranging the corpse and says he can't see them, citing the visions of the past from A Christmas Carol. Kenneth notes that this was the last piece he completed before the Carnage symbiote found him, noting that he'd been calling to it with his art and lamenting that it worked. Picking up a jar of blue grubs, Cletus asks what they are and Kenneth replies that they are purple emperor butterfly larvae. Although impressed that Kenneth managed to get his hands on the carnivorous caterpillars, Cletus scoffs at that Kenneth's methodology is too precise and controlled for his taste, lacking the joy of wanton slaughter. Asking where he got the caterpillars from, Cletus bursts into hysterical laughter when Kenneth awkwardly says he knows a guy at the zoo. Infuriated, Kenneth shouts at Cletus to stop laughing, grabbing a nearby scalpel and slashing Cletus' throat. As Cletus collapses, choking on his own blood, Kenneth initially blames him for not listening before fleeing from St. Estes. Cursing himself for murdering the one man who could stop Carnage, Kenneth trips and realizes he's still holding the scalpel, throwing it away. Cletus -- completely unscathed -- picks it up and teasingly say he dropped his paintbrush. Shocked, Kenneth realizes he's still in St. Estes and that Cletus was messing with him.
Realizing his idol has no intention of letting him leave, Kenneth asks Cletus what he is now if not Carnage. Smirking, Cletus exposits that his physical body perished some time ago, but that his consciousness was able to endure as a codex within the Symbiote Hive-Mind. The Carnage symbiote allowed Cletus to control it, bonding to a new host as part of a new scheme to seize power and sow chaos... until Iron Man showed up with an Extremis virus-infected symbiote-dragon he'd fused to one of his suits during Knull's invasion. Cletus had assimilated it into the Carnage symbiote, becoming more powerful than ever... but that was where he and the Carnage symbiote had a falling out: it had wanted to keep going, chasing godhood, but Cletus recognized that its hunger for power and destruction could never be satiated. Content with the power the Extrembiote afforded him, Cletus concluded that he'd seen the worst atrocities the real world had to offer and found them lacking compared to his gruesome imagination, so he used the Extrembiote to turn St. Estes into a place where anything his mind can dream up becomes real. As Kenneth asks what he intends to do about Carnage, Cletus retorts that as far as he's concerned Carnage can become a god of destruction out in the real world, while he's god of his own domain within St. Estes.
Pausing for a moment, Kenneth asks how Cletus feels being a bystander. Slamming the door he'd been holding open, Cletus stares furiously at Kenneth, who says that for the longest time Cletus and the Carnage symbiote were closer than mere host and symbiote, or even twin brothers. As Cletus menaces him with bladed tentacles, Kenneth says that Cletus was going to kill him anyway; continuing by saying that Carnage was meant to be Cletus and the symbiote together, with no distinctions between them, before asking Cletus who and what he is without the Carnage symbiote. Pondering Kenneth's question for a moment, Cletus grabs him by the throat and shouts that he's Cletus Kasady. As Kenneth says that people have forgotten what that means, forgotten the knives behind the teeth, Cletus notes that he's right and can see why Carnage let Kenneth tag along. Manifesting the Extrembiote into a hulking biomechanical suit of Carnage-ized Iron Man armor, Cletus tells Kenneth to hurry up and that they're going to give the world what it wants: the return of Cletus Kasady.
