Synopsis for "What If... The Invisible Woman Had Her Second Child?"
In the first story, Sue Richards carries her second child to term although it is slowly killing her. Dr. Morbius and Dr. Octopus are unable to save her life. The child is named "Suzy Too/II". Suzy is a succubus and is feeding off of those close to her. Franklin Richards knows what she is, but is unable to get anyone to believe him. Alicia Masters, Human Torch, and the Thing are eventually consumed by her while Reed Richards is in denial that she could be possibly killing anyone. Eventually Franklin pleads his case to Dr. Doom and he agrees to help stop her. Dr. Doom and Franklin return to capture Suzy, but Reed's denial fogs his mind and he renounces Franklin. Suzy shows her true form and consumes Doom, while Reed apologizes to Franklin and then is consumed by Suzy, too. Franklin turns on the portal to the Negative Zone and it traps Suzy in there where she is confronted by Annihilus; she expects to consume him. Franklin walks slowly out of the lab.

Synopsis for "What If... The Fantastic Four's Second Child Had Lived?"
In the second story, the second pregnancy poses no threat to Sue Richards and both mother and child are healthy. Eventually Mary Richards grows up to have mutant powers to physically heal and emotionally heal people. She becomes an activist for all minorities, mutants, etc. The government sees her as a threat and orders the Avengers to not attend any of Mary's rallies. All members quit after they receive the order from Captain America. Meanwhile Henry Peter Gyrich is hired to come up with a plan to bring down Mary. Gyrich puts on a Captain America costume and places instigators in the crowd. While disguised he attempts to kills Mary by stabbing her. This causes a riot and with her last bit of energy calms the crowd with her powers. Mary survives the stabbing and looks outside her hospital room to see a vigil in her honor.
Epilogue: Gyrich is upset that his plan failed and is then confronted by someone either dressed as Captain America or Captain America himself (eyes are glowing red).
